Just click on the above link, fill out the form and submit to receive your free Accidental Death Insurance. You can also print out the form and mail it in if you prefer. Your Certificate of Insurance detailing your benefits will be forthcoming in the mail.

With Guaranteed Acceptance there are no physical examinations and no health questions! Every eligible member who enrolls for coverage will be accepted-regardless of health or occupation.
